Community Member Impact – Medi-Cal CalAIM Housing Support

CCHI has partnered with Harmony Health since 2022 to support outreach and engagement for two key programs that help eligible community members enroll, maintain and use their coverage. Harmony Health helped us to organize our program of outreach, onboard and support each of our 18 participating community partners to take advantage of digital outreach in a way that complimented their unique workflows and processes. The team helped us every step of the way to ensure community partners could connect and reach members in their community in ways that complimented their existing workflows and best practices. The Harmony Health team has acted as an extension of our team and has been incredibly proactive in helping our partners reach and engage with their community.
